Director of Population Health & Health Equity
Salary: $120k – $140k
Nassau University Medical Center (NUMC) is a leading academic medical center located in East Meadow, New York. We are seeking a visionary and experienced Director of Population Health and Health Equity to lead our organization's efforts in improving community health outcomes and reducing health disparities. The ideal candidate will have a strong background in public health, healthcare administration, and social determinants of health.
In this role, you will:
- Develop and implement comprehensive strategies to address population health challenges and health inequities.
- Collaborate with community partners, healthcare providers, and policymakers to create and sustain health improvement initiatives.
- Analyze health data to identify trends, disparities, and areas for intervention.
- Design and oversee programs targeting specific health issues and at-risk populations.
- Lead a team of health professionals and coordinate cross-functional efforts.
- Secure funding through grants and partnerships to support health equity initiatives.
- Advocate for policies that promote health equity and address social determinants of health.
- Evaluate the effectiveness of programs and initiatives, making data-driven recommendations for improvement.
We require:
- Master's degree in Public Health, Healthcare Administration, or related field
- 7+ years of experience in population health management or related field, with at least 3 years in a leadership role
- Demonstrated commitment to health equity and experience working with diverse communities
- Strong understanding of social determinants of health and their impact on population health outcomes
- Excellent analytical skills and experience with health data analysis and interpretation
- Proven track record in program development, implementation, and evaluation
- Outstanding communication and interpersonal skills, with the ability to build relationships across diverse stakeholder groups
- Experience in grant writing and managing budgets
